Creatinine clearance calculator — Cockcroft–Gault

Estimate creatinine clearance using the Cockcroft–Gault equation — the estimate most drug labels specify for renal dose adjustment. For use by licensed clinicians.

Creatinine unit
Sex
Estimated creatinine clearance
87.5mL/min
Not body-surface-area normalised. Check the specific drug label — some specify Cockcroft–Gault CrCl and others specify eGFR, and the two are not interchangeable.
For use by licensed clinicians as a calculation aid only. It does not account for the individual patient context, and it does not replace clinical judgement or your local protocol. Verify any value before it informs a dose or a decision.

Equation used

CrCl (mL/min) = [(140 − age) × weight (kg) × 0.85 if female] / (72 × serum creatinine in mg/dL)

Still the estimate referenced by many regulatory drug labels for renal dose adjustment, which is why it persists alongside newer eGFR equations.

Common questions

Which body weight should be used in Cockcroft–Gault?
The original equation used actual body weight. In obesity, actual weight overestimates clearance and adjusted or ideal body weight is commonly substituted — the choice should follow the dosing reference you are applying.
Why use Cockcroft–Gault when CKD-EPI exists?
Because much of the renal dosing evidence and many approved labels were derived against Cockcroft–Gault. It is retained for dosing consistency, not because it estimates GFR better.
